Required Forms

 

To complete a GPS application, the Admissions Office requires the following forms be delivered to the applicant's current school.

 

 

Transcript Release Form

 

Please take this form to the current school after the completion of the first semester (January '10).

 

 

Teacher Recommendation Form

 

Please take the English and math teacher recommendation forms to the current teachers after the completion of the first semester (January '10).
